Welcome and thank you for considering Courtyard Nursing & Rehabilitation Center.

We understand that choosing the right skilled nursing facility can be overwhelming. Our staff works hard to make this decision and transition as simple as possible.

Courtyard Nursing and Rehabilitation is located in Houston, Texas, near several major acute care hospitals as well as many residential neighborhoods. Our center is a one hundred-twenty bed skilled nursing facility which provides twenty-four hour residential health services for persons who are no longer in an acute phase of illness but who continue to require frequent nursing observation, care and rehabilitative services.

We are proud to be operated by a Texas based company whose mission is to serve seniors across Texas.

Our purpose is to help chronically ill or disabled residents reach and maintain their greatest potential and to continue a quality living experience adapted to their individual health status and personal preferences. Our staff is dedicated to not only promote physical health, but also to enrich the lives of our residents by encouraging functional ability in all ways; physically, emotionally, mentally and socially.

At Courtyard our residents come first. We create a personalized plan of care that reflects each resident's specific needs. We get to know our residents, and develop individual service plans that reflect their preferences, likes, and dislikes. Our team pays special attention in the way they care for each resident, in order to provide the best experience possible. Our residents enjoy services delivered by caring, compassionate professionals, and families enjoy peace of mind knowing their loved one is getting the attention they need.

Your parent may need help with their activities of daily living, such as dressing, eating, bathing or using the bathroom. They may also need someone to cook meals, do laundry and keep their home clean, if they aren’t able to care for themselves in those ways anymore. And depending on their health status, they may need 24-hour supervision.
