Has anyone dealt with a denied health insurance claim for something really important (and expensive)? How did you handle it? Any programs or benefits for medical expenses?

Horrible place to be. Insurance denied my mom's hospital stay. I appealed it and sent in all her medical records. Took forever but they finally approved it. Check if your state has a health insurance assistance program - also look into hospital financial assistance programs.
