I'm meeting with my Mom's new doctor next week to go over her care plan. I want to make sure I don't miss anything important. She's still pretty independent but has some balance issues and mild memory problems. What are the key things I should make sure to address? I'm worried I'll forget something crucial in the appointment.

It's not something I like to think about, but it’s been on my mind. I wonder how others have prepared for this possibility. What steps have you taken to ensure your loved ones are still well cared for, even if you can’t be the one providing it? I’m curious about approaches others have taken and what options might be available. Share any insights or plans that helped ease your worries!

One week, it's her blood pressure, the next her arthritis acting up. I'm constantly playing catch-up. How do you all stay on top of everything and know when it's time to change things up care-wise?

He's got diabetes and early dementia. I'm totally overwhelmed trying to keep track of all his meds, appointments, and care needs. I tried making a spreadsheet but it's already a mess. Thought I would ask the community here.
