How do I have that tough talk about end-of-life wishes with my loved one?

I've been putting this off for months, but I know I need to talk to my mom about her end-of-life wishes. Just thinking about it makes my stomach churn. But I also know it's important to know what she wants. Any advice would be really appreciated.
