Does anyone have a good system or care plan I can follow to stay on top of everything with my dad?

Just moved Dad in with us after his last hospital stay. He's got diabetes and early dementia. I'm totally overwhelmed trying to keep track of all his meds, appointments, and care needs. I tried making a spreadsheet but it's already a mess. Thought I would ask the community here.

My mom is bedridden with dementia. Should I try to get her up daily?

How do you guys keep up with changes in your family member's health and adjust their care over time?

It seems like every time I turn around a new issue pops up with my wife's health. One week, it's her blood pressure, the next her arthritis acting up. I'm constantly playing catch-up. How do you all stay on top of everything and know when it's time to change things up care-wise?
